About the role

An exciting opportunity has arisen for an enthusiastic and motivated registered nurse to join the Dementia and Frailty Service within NHS Grampian.

Working collaboratively with primary care teams, patients, families and carers, the post holder will contribute to the delivery of person-centred care aimed at improving outcomes, promoting independence and reducing unnecessary hospital admissions.

Although based primarily within GP surgeries, the post holder will be professionally supported and operationally managed through Secondary Care Mental Health Services, ensuring access to specialist knowledge, clinical supervision and ongoing professional development.

The Role Will Involve

  • Providing support, education and advice to patients, families, carers and healthcare professionals.
  • Working collaboratively with GPs, Practice Nurses, Allied Health Professionals, Social Work and wider multidisciplinary teams.
  • Contributing to anticipatory care planning and complex case management.
  • Supporting service development and quality improvement initiatives.
