Responsibilities

Job Description

• Provide expert pharmaceutical care and advanced pharmacology consultation to patients and healthcare providers within the hospital setting.
• Develop and implement effective medication management plans for complex patient cases.
• Collaborate with multidisciplinary teams to optimize therapeutic outcomes and ensure safe medication practices.
• Educate patients and their families about medication therapy, potential side effects, and adherence strategies.
• Conduct medication reconciliation and review patient medication histories to prevent adverse drug interactions.
• Participate in the development and enforcement of hospital pharmacy policies and procedures.
• Support emergency care teams by providing timely pharmaceutical expertise in critical situations.
• Maintain accurate clinical documentation and ensure compliance with relevant regulations and standards.
• Stay current with the latest advancements in pharmacology and contribute to continuous improvement initiatives within the department.
